This study aims to quantify the costs of operating PSH—including both the costs of managing the property and providing supportive services—and examine what the implications of insufficient funding are for properties, staff, and residents. Working with a collaborative of seven affordable housing developers in the Bay Area, we analyzed data on operating and supportive services expenses to understand what influences the costs for 26 properties that include PSH units. We then explored how resident outcomes, including participation in resident services, on-time rent payments, and move outs, were associated with costs. We also conducted interviews and focus groups with 53 staff at the various organizations and properties, and held eight focus groups with 76 residents at different buildings to understand their experiences with services and how these resources impact their day-to-day lives. This report focuses specifically on the role of operating subsidies in managing PSH properties: future reports coming from this research study will explore other factors that influence the success of PSH.

Unsheltered homelessness is on the rise amid a systemic and widespread lack of affordable housing, supportive services, and livable wages. As the housing crisis worsens, homelessness has become increasingly visible and, as a result, increasingly dominant as a public concern.
Instead of addressing the issue’s root causes—a lack of housing and supportive services—many cities have leaned into punitive responses that criminalize homelessness, such as arresting people for sitting or sleeping in certain public places. But this approach is costly and ineffective. Police don’t solve homelessness, they only move it around—to other neighborhoods, jails, and emergency rooms—rather than connecting people with the housing and services they need.
What would it take to actually end homelessness for people living on the street? And how would that affect the time and resources police spend managing the problem without solving it? New data from a supportive housing program in Denver show what could happen when communities address the underlying causes of homelessness rather than continuing the status quo.

The role of public and nonprofit hospitals and hospital systems in supportive housing is not an obvious one at first glance. Traditionally, the role of nonprofit and public hospitals has been to provide primary healthcare and additional health services to the public, often to select “catchment” areas or neighborhoods. The extent of these institutions’ role in housing was limited to either providing a) short-term inpatient beds for medical or treatment services, or b) residences for nurses or other hospital staff. Why then are more and more nonprofit and public hospitals becoming involved in not only creating, but also providing and managing supportive housing?

The Keeping Families Together model turned the usual paradigm for prioritizing affordable housing on its head. Rather than targeting the most “stable” families, Keeping Families Together sought out families with the most complicated cases—those at greatest risk. Thanks to this approach, families once on the brink of crisis now have a permanent place to call home, as well as the services and support they need to stay together.

CSH selected the Center for Data Science and Public Policy (DSaPP) at the University of Chicago to develop a web-based data integration tool, which was completed in 2018. The tool connects county jail administrative data from the justice system to homeless system data, through communities’ Homeless Management Information Systems (HMIS).

People with mental health disabilities are vastly overrepresented in the population of people who experience homelessness. Of the more than 550,000 people in America who experienced homelessness on a given night in 2017, 1 in 5 had a mental illness. The proportion of people experiencing chronic homelessness with mental health disabilities was even higher—nearly 1 in 3. Despite this fact, the reality is that most people with mental illness fortunately do not experience homelessness: While about 20 percent of all adults in the United States have a mental illness, less than two-tenths of 1 percent of people in the country experienced homelessness on a given night in 2017.

The Colorado Coalition for the Homeless (CCH) has been investing in supportive housing since 1990. Since that time, this comprehensive community health center has developed nearly 1,700 units of housing, and is one of the country’s leaders in integrating health care and housing for a vulnerable population.
This webinar discusses how CCH finances its capital development; how they plan, design, and manage multiple projects simultaneously; how they integrate housing and health care services; and how they include property management staff in a coordinated approach to care. The Colorado Coalition for the Homeless created the Denver Housing First Collaborative (DHFC) in 2003 with funding provided by a collaboration of federal agencies. The DHFC involved CCH as the lead agency, Denver Department of Human Services (DDHS), Denver Health (DHHA), Arapahoe House, the Mental Health Center of Denver (MHCD) and the Denver VA Medical Center. The DHFC is designed to provide comprehensive housing and supportive services to chronically homeless individuals with disabilities. Initial federal funding created the capacity to house and serve 100 chronically homeless individuals. The program uses a housing first strategy combined with assertive community treatment (ACT) services, providing integrated health, mental health, substance treatment and support services.

Everyone needs safe, decent, stable housing. For some of the most vulnerable people in America — people with mental illness, chronic health conditions, histories of trauma, and other struggles — a home helps them to get adequate treatment and start on the path toward recovery. But some conditions make it difficult for people to maintain a stable home without additional help. Supportive housing, a highly effective strategy that combines affordable housing with intensive coordinated services, can provide that needed assistance.

Supportive housing programs are proposed as a way of increasing housing access and stability for the chronically homeless, improving access to needed services, and decreasing vulnerability to HIV and other diseases. Little is known about residents’ understandings of and experiences with different models of supportive housing and how they fit within residents’ broader strategies to maintain housing. We conducted semi-structured interviews with 23 residents and 10 service providers from nine different supportive housing programs in Hartford, Connecticut. Data analysis explored residents’ perceptions of and experiences with supportive housing programs in the context of strategies to access resources and receive emotional, financial, and other forms of support.

Part of New York State’s Homelessness Action Plan includes an investment of new supportive housing resources and services over the next five years to address vulnerable populations experiencing homelessness. Time and again, taking a narrow view of health care has proven ineffective in producing meaningful change. Yet the current
thrust of health care reform remains firmly focused on traditional health care services. Reforms such as care coordination models and patient centered medical homes are necessary but insufficient for homeless populations with complex problems. Pairing such reforms with supportive housing is more likely to result in lasting health improvements and reduced costs.

This issue brief outlines ways in which Medicaid can support integrated strategies and, based on telephone interviews with key informants, profiles three current initiatives that illustrate distinctly different approaches to linking Medicaid and supportive housing. The three initiatives include one launched by a city (Philadelphia), one by a state (Louisiana), and one by a Medicaid MCO (Mercy Maricopa Integrated Care in Phoenix, Arizona). They target special populations including homeless individuals, people with a wide range of disabilities, and adults with mental health and/or substance use problems.

These five steps will help you build a case for why and how Medicaid can be structured at the state and local levels to pay for services in permanent supportive housing. When building a case you need to know some basics of Medicaid and the types of services you want Medicaid to cover. in order to convince Medicaid administrators and other health care payers to support your efforts, you need to have evidence of need for and impact of supportive housing and you need a coalition of many stakeholders at your side.
