CLPHA Housing Is Summit 2022: A Sustainable Approach to Addressing the Social Determinants of Health
The Columbus Metropolitan Housing Authority (CMHA) has formed a partnership with the Healthcare Collaborative of Greater Columbus (HCGC) and their Central Ohio Pathways HUB to provide supportive services to all 14,000 CMHA-assisted households. HCGC’s HUB is a nationally certified program focused on addressing the Social Determinants of Health in an effort to improve health outcomes for at-risk populations.
