A Summary of Results from the MIHOPE and MIHOPE-STRONG Start Studies of Evidence-Based Home Visiting
A healthy birth and positive experiences in early childhood can promote health and development. One approach that has improved outcomes for children and their parents is home visiting, which provides individually tailored support, resources, and information to expectant parents and families with young children. This brief summarizes recently published reports from two national studies of evidence-based early childhood home visiting: the Mother and Infant Home Visiting Program Evaluation (MIHOPE) and MIHOPE-Strong Start.

Island School is one of 247 “community schools” in New York. These are regular public schools, with a twist. They have longer days and longer school years: Island stays open 12 hours a day, six days a week, including spring and winter breaks as well as the summer. A psychologist makes weekly rounds. A dentist comes by regularly. So does an optometrist, and students who need glasses get them free.

Health departments provide public health protections in a number of areas, including: preventing the spread of communicable disease, ensuring food, air, and water quality are safe, supporting maternal and child health, improving access to clinical care services, and preventing chronic disease and injury. In addition, public health departments provide local protections and services unique to their community’s needs.

During this All In webinar, Caroline Fichtenberg of SIREN reviewed the current landscape of assessment tools and outcomes measures for social needs. Karis Grounds of 2-1-1 San Diego explained how they incorporated social needs assessment into their Risk Rating Scale, which helps them better serve clients while showing the impact of their services.

A growing recognition of the importance of social determinants of health (SDOH) has led to a proliferation of screening and assessment tools, but there are currently no national standards for how to systematically capture and address the non-health needs of patients. Many social issues stem from a history of unstable, unaffordable, and poor-quality housing. Research shows that housing is the first rung on the ladder to economic opportunity for individuals and that a person’s access to opportunity is intrinsically linked with that of the community at large. As the gap between rents and incomes widens, it is critical that professionals in fields outside housing—including health, education, and economic development, among others—understand its central importance.

Health care payment and delivery models that challenge providers to be accountable for outcomes have fueled interest in community-level partnerships that address the behavioral, social, and economic determinants of health.We describe how Hennepin Health—a county-based safety-net accountable care organization in Minnesota—has forged such a partnership to redesign the health care workforce and improve the coordination of the physical, behavioral, social, and economic dimensions of care for an expanded community of Medicaid beneficiaries.

Homelessness among children is correlated with developmental delays, fair or poor health, and high healthcare utilization. Associations of homelessness specifically among infants younger than 12 months, however, are unknown. This study evaluates homelessness during infancy as a risk for adverse infant and maternal health and hardship.

Although public-private partnerships (PPPs) have attracted practitioner and academic interest over the last two decades, there has been no attempt to integrate the general and health management literature to provide a holistic view of PPPs in healthcare delivery. This study analyzes over 1,400 publications from a wide range of disciplines over a 20-year time period. It synthesizes formerly dispersed research perspectives into a comprehensive multi-dimensional framework of public-private partnerships, and in so doing, provides new directions for further research and practice.

Through the coordinated care model, those paying for health care get a better value and health plan consumers get higher quality care at a price we can all afford. And Oregonians are experiencing improved, more integrated care. With a focus on primary care and prevention, health plans and their providers using the coordinated care model are able to better manage chronic conditions and keep people healthy and out of the emergency department.

Increasingly, a wide range of entities—housers, educators, cities—are using data sharing as a way to increase effectiveness across systems. Data sharing exemplifies this community of practice’s intent to align systems to improve life outcomes for low-income children. We have many robust examples on the local level and this panel will discuss what data systems beyond local partners can look like and accomplish.
