A more regional approach to care is needed – one that involves coordinated, person-centered healthcare with robust connections to social services and community resources. An innovative infrastructure to do just that is underway in four communities across New Jersey: Trenton, Camden, Newark, and Paterson. The Home Preservation Initiative (HPI) for Healthy Living seeks to improve asthma outcomes related to unhealthy housing in five neighborhoods in West Philadelphia. By combining home repairs and community health worker home visits, HPI aims to significantly reduce emergency department visits and hospitalizations due to pediatric asthma. For these primarily African-American communities, substandard housing, unemployment, low wages and a lack of education are barriers to the overall health and well-being of residents. Using outcome data, the collaboration will show health care cost savings, aiming to make a strong case for Medicaid reimbursement for home repairs.

The Addressing Healthcare’s Blindside in Albuquerque’s South Side (AHBASS) collaborative participated in the BUILD Health Challenge’s first cohort with the intention of “making the healthy choice, the easy choice” for community members. Now you can learn about their strategies, approaches, application of the BUILD principles, and outcomes in this new case study documenting their work. Follow along in this case study and see how this Albuquerque, NM, based team addressed chronic disease and self-management in their community. Together, they established a multi-sector collaboration that resulted in a referral tracking system and a Mobile Farmers Market. In addition, the team implemented the Healthy Here Wellness Referral Center, an integrated chronic disease management referral system to link clinics to community resources in order to improve health outcomes.

It is intended to familiarize state Medicaid, public health, and other state policymakers with the funding streams of other agencies, and sketch out a continuum of options to help states coordinate funding to better serve the needs of low-income populations. Because this brief focuses on services for adult Medicaid beneficiaries, it does not address many of the funding sources available for children’s services. However, existing efforts to pool funds for children and youth—notably by the Commonwealth of Virginia—could prove instructive for states seeking to launch such an effort for adults.
